For long-term care ombudsman programs
“What is on file about this facility, and what should a bed cost here?”
Ombudsman staff need two things about a named facility: its public record and the local going rate. Every provider profile shows the license, the inspection and deficiency record where the state or CMS publishes one, ownership where it is disclosed, and the local median for its care type. The record is never removed or softened because a provider asks.
What to hand a family
- Cost of care by state and cityThe local median and range for the facility's care type, to put a quoted rate in context.
- Tour checklistA printable page of questions for a resident's family to ask on a visit or at a care conference.
- Compare citiesWhen a transfer to another county or state is on the table.
Links that remember where a family came from
Paste these into a resource page, a newsletter or an email. The tag on the end tells us a family arrived through your link, and nothing more: no cookie, no account, no information about the person. If you want to see how many of your families used the site, ask and we will send the count.

State fact sheets
The state fact sheet carries the nursing home and assisted living medians and the state's rank, useful in a resident council or a legislative briefing. Every state has one, and each prints on a single page from the browser. Pick a state.
Put a cost table on your own page
The embed gallery generates one line of code for a city cost table, a city against state against national comparison, a licensed provider count or a care type cost card. The widget updates itself when the data updates and carries one attribution link back to the source page. No registration and no tracking script.
Frequently asked questions
Do you rank facilities by inspection results?
No. Inspection and deficiency data is shown as the record surveyors wrote, with its date and source, and the site says plainly that a count is not a quality verdict. Directory pages list providers by verification date, not by a score.
Can a facility pay to remove a deficiency from its profile?
No. Nothing on a profile is removed for payment. A provider can claim its listing to correct contact details and report its own price, and that is marked as owner-reported.
