For hospital discharge planners and case managers
“We have two days. What does skilled nursing cost near the patient, and who has a bed?”
A discharge gives a family days to make a decision that normally takes weeks. The city pages for nursing homes and home health put the local median beside the licensed providers in that city, with CMS star ratings and inspection records where they exist, so the family can be handed one link instead of a stack of brochures.
What to hand a family
- Cost calculatorThe monthly figure for skilled nursing against home health at the patient's city, in one screen.
- Tour checklistQuestions for the family to ask on a same-day visit, printable at the nurses' station.
- Care quizThree questions that point a family toward the care type to research. Not an assessment.
Links that remember where a family came from
Paste these into a resource page, a newsletter or an email. The tag on the end tells us a family arrived through your link, and nothing more: no cookie, no account, no information about the person. If you want to see how many of your families used the site, ask and we will send the count.

State fact sheets
The state fact sheet is the single page to hand a family who asks what this will cost after the Medicare days end. Every state has one, and each prints on a single page from the browser. Pick a state.
Put a cost table on your own page
The embed gallery generates one line of code for a city cost table, a city against state against national comparison, a licensed provider count or a care type cost card. The widget updates itself when the data updates and carries one attribution link back to the source page. No registration and no tracking script.
Frequently asked questions
Does the site show bed availability?
No. It shows every licensed provider in a city with its public record and contact details. Availability changes hourly and only the provider can confirm it.
Is there anything the family has to sign up for?
No. Every cost figure and provider list is open. A family can ask for a shortlist by email if they want one, and that form is optional and separate.
