CMS currently lists 84 Special Focus Facilities and 443 SFF candidates across 49 states and territories in our directory (Texas has the most flagged facilities at 37).
What the numbers mean
Counts copy CMS Special Focus Status labels only: SFF or SFF Candidate.
This is a designation count, not a ranking of “worst” facilities.
Monthly CMS refreshes can move facilities on or off either list.

Methodology
Copy CMS Provider Information Special Focus Status exactly: SFF or SFF Candidate.
Count nursing homes per state. Blank/null status is excluded.
Language stays designation-only. We do not invent severity adjectives.
Limits
SFF and candidate lists refresh monthly with CMS. This page reflects the last ingested Provider Info snapshot.
Candidate status is not the same as current SFF program membership.
