Licensed capacity

Beds or units on the license. A capacity, not an occupancy.

License status

The status word the state registry uses, reproduced exactly. We do not translate or grade it.

Published rating

Whatever the publishing regulator published, on the scale the regulator used. Nothing here is rescaled to five stars.

Not a like-for-like comparison. We hold published rating for some of these providers and not for others, and we cannot yet say whether the gap means a clean record or a file we have not read. Treat the difference between these columns as unknown, not as a finding.

Inspection citations

How many citations are on record, and how recent the newest one is.

Fines and payment denials

Enforcement actions on record, with the total fine amount where the source publishes one.

Nurse staffing hours

Payroll-based nurse hours per resident per day, against the median for the same state.
