How hospice costs work

For people enrolled in the Medicare hospice benefit, covered hospice services are generally about $0 out of pocket: nursing, aides, medications related to the terminal illness, medical equipment, and grief support. That is a benefit design fact, not a local price quote for the United States.

  • Room and board is separate. If your parent is in a nursing home or assisted living while on hospice, Medicare hospice does not pay the facility rent. The family, Medicaid, or long-term care insurance usually covers that.
  • When families do pay: care unrelated to the hospice diagnosis, private-duty extras the hospice does not cover, or hospice outside Medicare (rare private-pay arrangements). Always ask what is billed to whom.
