Assisted living in Arizona: care levels and ALTCS

Arizona's median of $6,250 a month matches the national figure almost exactly. Two features of the state's system decide most of the practical questions: the three licensed levels of care, and ALTCS, the Medicaid program that pays for assisted living more readily than most states.

Three levels of care on the license

The Arizona Department of Health Services licenses assisted living homes (ten or fewer residents) and assisted living centers (eleven or more), and each license names the highest level of care the community may provide: supervisory care, personal care, or directed care. Directed care is the level for residents who cannot direct their own care, which is what memory care requires. A community licensed only for personal care cannot keep a resident whose dementia progresses, so match the license level to the diagnosis, not just today's needs.

Where the price comes from

The $6,250 median is a modeled figure from 2025 survey data, with a range of about $5,966 to $6,793. Scottsdale and the East Valley run higher, Tucson and the rural counties lower. Arizona has a large number of small assisted living homes, which often price below the centers; the city pages below carry local medians.

ALTCS pays for assisted living

Private pay is the norm. Long-term care insurance and VA Aid and Attendance cover part of the bill for those who have them. Medicaid is the piece that varies by state, and in every state it pays for care services, never the room and board portion of an assisted living bill.

The Arizona Long Term Care System (ALTCS) is the state's Medicaid long-term care program, and unlike most states it routinely pays for care in assisted living for members who meet the nursing-home level-of-care test and financial limits. The member pays room and board from income. Not every community accepts ALTCS, and those that do may limit ALTCS beds, so ask up front. Applications go through AHCCCS, the state Medicaid agency, and take several weeks.
